You are undergoing or awaiting diagnostic tests, test results, or medical investigations, or a diagnosis had not been made, or
Where you have been prescribed medication, undergone surgery or any procedure, or received therapy or rehabilitation in the two-year period before, or
You are required to have regular check-ups by a doctor or nurse.
